Application Process

A Microsoft Word version of the application may be downloaded by clicking HERE.  The completed application may be e-mailed to hr@pemhs.org, faxed or mailed to the Human Resources address listed below.  Use your mouse to click on each grey area of the application.  The lines will wrap if necessary to accommodate your information.

If you prefer the application may filled out at any of our locations. Applications will be forwarded to Human Resources to ensure applicants meet the minimum qualifications for the position applied for.  Applications shall be forwarded to appropriate managers for consideration.
