A collaborative partnership between the Pinellas County Sheriff’s Office, Personal Enrichment through Mental Health Services - PEMHS, and the Juvenile Welfare Board

About Our Program

H.O.M.E. Navigation provides intensive wrap-around case management that focuses on decreasing the at-risk behaviors of juveniles involved with the Department of Juvenile Justice, as well as strengthening the overall sustainability of their families.  This results in the greater stabilization and safety of the family and juvenile within the community.  Our goal is to reduce barriers, provide linkage to services, and strengthen families in a variety of domains.

Eligibility and Referral

  • Juveniles, and their families, residing in Pinellas County, with five or more felonies, and or those on the high offense list, as identified and referred by the H.O.M.E. Taskforce
  • Direct referrals from HOME Taskforce or the juvenile’s JPO
  • Any juvenile that is currently connected with the Department of Juvenile Justice who is on intake status or probation

WORKING WITH H.O.M.E. NAVIGATION

H.O.M.E. Navigation is a voluntary program for families that are in need of services.  The H.O.M.E. Navigator will partner with the parent and/or Youth to develop a support plan encompassing the needs of the family or individual.  This is a voluntary program and the length of stay is dependent on the needs of the family.

SERVICE LINKAGE

H.O.M.E. Navigators have the ability to connect the entire family with an array of different resources and services within the community.

  • Financial Assistance Programs
  • Financial Literacy Programs
  • Youth and Adult Employment Programs
  • Supportive Programs
  • Early Learning Coalition-Childcare
  • Educational Support
